New PAtient INFORMATION AND FORMS
Please download and complete the new patient forms and bring them with you to your first appointment. If you are unable to download these forms, please contact our office. We can either mail, email, or fax these to you prior to your appointment upon request. You can also arrive at least 15-20 minutes before your appointment to complete these forms.
If you are being seen only for acupuncture, you will need to call our office so we can send you our Acupuncture New Patient Paperwork.
Other items required at the time of your first appointment are a most up-to-date insurance card and your Referral if you have a HMO.
Other items that are not required, but can be helpful for your first appointment are copies of any recent lab tests and details on any supplements and medications that you are taking.
WHAT ARE THE DIFFERENCES BETWEEN OUR SALEM, NH OFFICE AND BOSTON, MA OFFICE?
Naturopathic Doctors are licensed as primary care doctors in the state of New Hampshire, allowing them to provide physical exams, order lab tests, and prescribe medications. Naturopathic Doctors are not licensed yet in the state of Massachusetts, so visits in our Boston office may differ from those in our Salem office.
Since the licensure is not yet established in Massachusetts, visits in our Boston location are not covered by insurance. It is possible to use a Flexible Spending Account or Health Savings Account, but this will depend on the details of your account.
what to expect during a visit
Boston Office
Initial Visit
Typically between 60–90 minutes
During this visit, Your doctor will go through the details of your health concerns to understand the root cause of your symptoms. Your doctor will use this information to determine if any lab tests are needed and to develop a plan that may include dietary recommendations, supplements, and homeopathic remedies.
General Follow Up Visit
Typically 50 minutes
During this visit, your doctor will go through the details of any changes that have occurred and will determine whether the initial recommendations have been helpful and if they need to be continued. If any lab tests were done, results are usually discussed during the visit. Follow up visits are typically once a month, particularly in the beginning, so your doctor can assess and evaluate any changes.
LIMITED Follow Up Visit
Typically 20-25 minutes
This visit is usually recommended when less time is needed compared to a General Follow Up Visit.
PLEASE NOTE: Our offices have a fee of $100 if cancellation is not provided within 48 hours and/or if there is a no show. Please provide at least 48 hours notice for any cancellations or if you need to reschedule your appointment. This applies to new patient visits and follow up visits.
New Hampshire OFFICE
INITIAL VISIT

Follow up visits are typically once a month, particularly in the beginning, so your doctor can assess and evaluate any changes.
LIMITED FOLLOW UP VISIT
Typically 15-30 minutes
This visit is usually recommended when less time is needed compared to a General Follow Up Visit.

GROUP VISITS AND WELLNESS CLASSES
At our New Hampshire office, we offer a variety of wellness classes throughout the year in a group setting for our patients. Contact us if you would like to receive updates on our classes. We send updates through our monthly newsletter. These classes are usually held during the lunchtime hour and/or in the early evening.
METHODS OF PAYMENT
Both of our offices accept cash, check, and major credit cards. For our Salem, NH office, we do accept insurance (CIGNA, Harvard Pilgrim/United Healthcare, Anthem BC/BS). It is highly recommended to call your insurance company to know the details of your coverage. We do not accept insurance for our Boston office, but a Flexible Spending Account or Health Savings Account may be used, depending on your account.
Our doctors are providers of CIGNA, Anthem BC/BS, Harvard Pilgrim, and United Healthcare. Coverage is dependent on your plan. We are not responsible for checking the details of your plan, so we highly recommend calling your insurance company to know what is covered and what is not covered. If you have Anthem BC/BS, you will need to make sure you have a "Naturopathic Rider" on your insurance plan. If you have a HMO plan, you will need a referral to be sent to our office prior to your appointment. If we do not have this referral, we will need to reschedule your appointment.
If you have acupuncture coverage and want to use your insurance for acupuncture visits, we recommend that you call your insurance to know how many visits are covered. At this time, we only accept Anthem BC/BS for acupuncture. If you don't have Anthem BC/BS, but you have acupuncture coverage, you can look into reimbursement options with your insurance company.
Each insurance company and plan have preferred lab companies. We ask that you become familiar with which lab your insurance company and plan prefers. For example, the main lab companies used are Quest Diagnostics and Lab Corp.
At this time, we are not providers of Medicare. We do offer a discount for seniors.
